Derbarl Yerrigan's Home and Community Care (HACC) program aims to provide a comprehensive, coordinated and integrated culturally appropriate service to the Aboriginal and Torres Strait Islander frail aged people and young people with a disability with in the Perth Metropolitan area. |

The Health Promotion team is located at East Perth, however the team services all of the DYHS sites. The Health Promotion team is a state initiative that aims to increase the awareness of the importance of good health and wellbeing in the prevention of diseases and issues relating to nutrition, physical activity, chronic disease, smoking, sexual health and other issues as identified by the community. The Health Promotion team helps to identify the needs of the community, and develops culturally appropriate strategies to support the implementation of programs and projects, in and around DYHS. The Health Promotion team’s main task is to support the many programs and projects running across the organisation which help individuals, families and community groups. The Health Promotion team support these programs and projects to promote beneficial lifestyle practices and the prevention of disease.The Health Promotion team has designed and developed program brochures, promotional merchandise and in-house literature aimed at spreading the good health message; whilst raising the profile of DYHS within the Perth region. All materials developed are culturally-appropriate and as locally specific as possible. The Health Promotion team is involved with many community events and organisations such as NAIDOC, Have-A-Go Day, Survival Day, Nicky Winmar Cup, North and South Metro Public Health Units, schools, various fundraising events for non-profit organisations, Department of Sport and Recreation and Seniors Recreation Council of WA (Inc) just to name a few.
