Stolen Generation - Bringing them Home |
The Stolen Generation program provides a comprehensive service to improve the social and emotional well-being of Aboriginal people in the Perth metropolitan region. |
Maternal and Child Health Program |

Home and Community Care Program |
Derbarl Yerrigan's Home and Community Care (HACC) program aims to provide a comprehensive, coordinated and integrated culturally appropriate service to the Aboriginal and Torres Strait Islander frail aged people and young people with a disability with in the Perth Metropolitan area. |
| Chronic Diseases |

Chronic Disease, consists of a Registered Nurse for the Chronic Disease program that is based at the East Perth Site. The Chronic Disease program looks at targeting chronic diseases, such as diabetes and heart disease through working with the Health Promotion team to target prevention; working with the clinic staff at all three sites to complete Adult & Chronic Disease Health Checks; and through the delivery of a weekly Cardio Rehabilitation Program (Heart Health). |
