Derbarl Yerrigan's Home and Community Care (HACC) program aims to provide a comprehensive, coordinated and integrated culturally appropriate service to the Aboriginal and Torres Strait Islander frail aged people and young people with a disability with in the Perth Metropolitan area. HACC service aims to support our people to be more independent at home and in the community, enhancing their quality of life by assisting with tasks that they are physically unable to perform themselves. Other Strategies and Aims:
Eligibility:Eligibility for the DYHS HACC program is assessed through the use of the HACC assessment forms and is based on the criteria. This determines the level of need by an individual. Service Delivery:Once aboard the HACC program, you will then be appointed a HACC worker who will establish a convenient ongoing appointment. (If you are not home for your appointments for an extended period of time your HACC service can be cancelled). Personal Care:HACC workers can assist with essential tasks of daily living such as Bathing, getting dressed, toileting, medication reminder calls. Social Support:This can be provided either in the home or in at the Day centre. This can include company to appointments helping with paper work, shopping and banking. Respite Care:Respite care assistance is provided so that the DYHS HACC client carers may have relief or time out from their caring role Allied Health:Allied health service include Podiatry and Physiotherapy. Referrals can be made to social workers, DYHS Ear and Eye Health programs or the dentist. Click Here to Download the brochure
