Derbarl Yerrigan's Home and Community Care (HACC) program aims to provide a comprehensive, coordinated and integrated culturally appropriate service to the Aboriginal and Torres Strait Islander frail aged people and young people with a disability with in the Perth Metropolitan area. |
| Heart Health “For our people, By our people” |

In June 2009, DYHS in conjunction with The Heart Foundation implemented the heart health program, “For our People, By our People”. This program is run weekly and covers a variety of topics including heart health, heart medications, healthy tucker and healthy weight, oral health, blood pressure, cholesterol control, quitting smoking, managing stress and emotions, benefits of physical activity and diabetes. Guest speakers are also invited to speak to the group on relevant issues. A Chronic Disease nurse, Cardiac Rehabilitation nurse, exercise physiologist, physiotherapist, GP’s, nurses and health workers all work together to provide the best care for the heart health clients. Also, every month a full day workshop is held. |
